this changes how we make appointments for you, and who you may see for your health and wellbeing needs. in essence, we have tried to provide a system of care for you with an appropriate clinician to meet your clinical need. we are offering continuity of care (so you don't have to keep repeating your story to a new face at each appointment), and are ensuring that if you have a clinical need to be seen urgently, you will. it may mean that if you are usually fit and well you rarely see 'your gp'. this is because 'your gp' may be looking after those patients that need to see them for ongoing management of conditions, and offering them longer appointments where necessary as these are the patients who are not usually fit and well and need our care and expertise the most. we are also encouraging our patients to self-care at home, or by visiting their pharmacist first. there are websites and information below to help find out more about this. the changes we have made should see an improvement in our ability to deliver you and your family with a sustainable service that meets your healthcare needs. all the staff are working very hard to make this change a success, and we welcome constructive feedback. please bear in mind that change is hard work, and takes a while to get used to. we have new staff who are being trained, new appointment systems, new ways of doing things, and a lot of other changes. unfortunately the changes are something that we have had to do. in our recent letter to our patients (see below 'letter to all patients april 2017') we wrote about how we were struggling to replace our doctors who had left the practice due to retirement or relocation. nationally, there is a recruitment shortage of gps, doctors, nurses and the healthcare workforce is struggling as a result. to overcome our problems, and continue to meet your healthcare needs, we have launched a new way of working. we have recruited clinical pharmacists and specialist nurse practitioners we have identified locum gps that we can book on a regular basis (rather than as a one-off, where they do not 'know' the patients or the surgeries) we have analysed our patient database to identify patients who would benefit from routine and timely care with the same healthcare team member (continuity of care) we have set up our 'triage hub' so that all patients requiring 'on the day' clinical advice or review are spoken to by a member of the clinical team patients who require continuity of care for an episode of treatment are given slightly longer appointments with their named clinician, and this is usually with a clinician employed on a long-term contract with the practice (not a 'locum'). patients who are not being seen for an episode of care (those that 'just need to see a nurse or doctor') will be offered appointments with a relevant clinician after an assessment made by triage. we have signed up to be part of a 'place based care pilot' with the 2gether team. as part of this we have a mental health & wellbeing practitioner offering telephone call-backs and appointments at the surgery. we are part of the nhs england waveii clinical pharmacist in gp practices scheme and look forward to welcoming another clinical pharmacist to join our team soon. appointments will be offered for medication reviews and queries, and changes to medications required (for example after a patient leaves hospital) will be carried out by this team. in essence, we have tried to provide a system of care for you with an appropriate clinician to meet your clinical need. we are offering continuity of care (so you don't have to keep repeating your story to a new face at each appointment), and are ensuring that if you have a clinical need to be seen urgently, you will. it may mean that if you are usually fit and well you rarely see 'your gp'. this is because your gp may be looking after those patients that need to see them for ongoing management of conditions, and offering them longer appointments where necessary as these are the patients who are not usually fit and well and need our care and expertise the most. how you can help!
